Appointments

Dr. Ford sees cases on a house-call basis in northeast Ohio. A typical appointment will last one and half to three hours.

You will be asked to complete a pre-appointment questionnaire detailing the pet's behavior history. It is very important that the client fills this questionnaire out completely and as thoroughly as possible. The questionnaire must be returned within one week of the appointment.

The appointment includes a discussion of the pet's history, diagnosis, prognosis, treatment plan and a behavior modification demonstration with the pet.

After the appointment the client will receive a typed evaluation and treatment plan which will also be sent to the client's veterinarian. Limited follow-up phone calls and emails are also included.

Cost

$200 - includes appointment and follow-up questions via phone or email
$50  - 30 minute re-check appointment if necessary
